Requesting a Transcript

All transcripts and diplomas for the Maurice H. Kornberg School of Dentistry are generated by Temple University Office of the University Registrar. In the event of technical issues obtaining your transcripts, placing an order, completing the identity validation process, you will need to contact the Office of the University Registrar at:

Official transcript: An official transcript displays the terms you attended, graded courses, academic programs, and other relevant academic information. Current students, former students and alumni can place an order for an official paper transcript or an electronic transcript (eTranscript).

Unofficial transcript: Office of the University Registrar does not provide copies of unofficial transcripts. Unofficial transcripts are available for review in Self-Service Banner for current students and alumni.

Replacement Diploma: Replacement diploma orders are generally processed with 21 business days after we receive the order. If your degree was awarded prior to July 1, 2019, you will also receive a complimentary digital diploma with your order.

Professional Licensing

The Office of Academic Affairs may coordinate the completion of professional licensure forms. Due to the volume of request, licensure submissions will be processed within in 4-7 business days however, state licensing agencies may take 4-6 weeks for processing. Please note that not all states immediately update the applicant’s account.

Professional Licensing: Forms must be submitted by the student or alumna/us. The completed forms will be sent from the Office of Academic Affairs by email (or mail) to the agency. Please send documents to . Submitted forms that are missing information will have a delay in completion time. 

Verification of Nitrous Oxide Certification: Individuals who require proof of completion of the Nitrous Oxide Certificate course should e-mail the . Please list your full name (as it appears on your student record), contact information, date of attendance, and addresses to which the letter(s) should be sent. 

National Board Scores: National Board Scores are available through the American Dental Association (ADA) and are not stored at the School of Dental Medicine after graduation. To locate the order form for National Board score reports, visit the Joint Commission on National Board Dental Examinations.

Enrollment Verification

A verification provides information regarding a student's enrollment status, dates of attendance, degrees sought or awarded, and program of study. 

Verification of Attendance for a DDS/DMD Alumni: 

Alumni who need a verification form completed or request a custom verification letter should follow these steps:

  1. Using your TUportal login, sign into the Verification app to submit your request.
  • Don't have an active TUportal account? Former students and alumni can activate it through the identity validation process then sign into TUportal.
  1. Select the request type. See above for descriptions.
  2. Enter the recipient information. The Office of the University Registrar does not verify the recipient information.
  3. Click the Submit button.
  4. You will receive a confirmation upon submission, and your request will be processed in the order received.

Verification of Attendance for a MS or Resident Alumni: Alumni requiring verification of attendance, listing the degree award date, degree received, and dates of attendance should the Department of Graduate Education. Please list your full name (as it appears on your student record), contact information, date of birth, degrees received, date of attendance, and addresses to which the letter(s) should be sent. Due to the volume of requests, please allow 4-7 business days for processing. ​

Degree Verification

A verification provides information regarding a student's enrollment status, dates of attendance, degrees sought or awarded, and program of study. This information is often needed by lenders, insurance carriers, sponsors, etc., to determine eligibility for loan deferment and continuation of in-school benefits. 

What is an eVerification?
Current or past students can request a current term enrollment verification or a degree verification to be sent directly from the Office of the University Registrar. The eVerification letter shows the student's recent enrollment dates, time status, degree information, and anticipated graduation date. The letter can be delivered as a digital document to the agency by email.

Verification for DDS/DMD
Optionally, insurance agencies and employers wishing to verify the DDS/DMD education of a Maurice H. Kornberg School of Dentistry graduate can contact the National Student Clearinghouse for an enrollment or a degree verification.

Verification of Residency: Insurance agencies and employers wishing to verify the residency education of a Maurice H. Kornberg School of Dentistry graduate should contact the Department of Graduate Education via email at . Insurance agencies or employers must submit their request verification AND a release signed. Verifications cannot be completed without the signature of the alumna/us. Past residents please list your full name (as it appears on your student record), contact information, date of birth, residency program attended (i.e. Endodontics), date of attendance, and address(es) to which the letter(s) should be sent.  Due to the volume of requests, please allow 4-7 business days for processing. 

ADEA PASS

PASS Applicants: Students may request an Institution Evaluation Form [IEF] from the Kornberg School of Dentistry. IEF’s are required for residency applications through the Postdoctoral Application Support Service (PASS) but may also be required for non-PASS programs. Please e-mail the 

The form will be submitted electronically, and the information contained within will be used by the Office of Academic Affairs to complete the Institution Evaluation Form. If your program does not participate in PASS, please indicate that on the form, and provide the addresses of the institutions.
